Applications are hereby invited from suitably qualified candidates for admission into Obafemi Awolowo University Teaching Hospitals Complex Basic School of Nursing, Wesley Guild Hospital, llesa for a – 3 year General Nursing Course, commencing in September, 2013.

Applicants must possess:

WAEC, SSCE/GCE OR NECO SSCE/GCE, 5 Credits (at not more than two- sittings) in English Language, Mathematics, Physics, Chemistry and Biology. He/She should be at least seventeen and half (17%) years old by October, 2013. Applicant must note that combination of NECO and WAEC results is ACCEPTABLE.

Application form is obtainable from Obafemi Awolowo University Teaching Hospitals Complex, School of Nursing, Wesley Guild Hospital, llesa at a cost of Five Thousand Naira (#5,000.00) only payable by cash to the School’S Account stated here under: Skye Bank Plc: Wesley Guild Hospital, llesa Account No: 1770751539

The Payment entitles the applicant to:

i. Application form

ii. Scratch card for the result

iii. Past question paper

Applicant should note that no part of the form be detached before submission.

Passport Photograph:

Identical passport photographs taken within six months to examination should be attached to forms.

Registration for the Post UTME Screening of YABATECH will close on 19th August, 2013.

All candidates should visit the website of Yaba College of Technology at www.yabatech.edu.ng Click on “Post UTME Application” and complete the basic details form.

Applications are to pay One Thousand, Five Hundred Naira (N1,500.00) for the Post UTME and result checking on-line using ATM Card by clicking on PAY NOW (Please note that transaction charges is N300.00) Kindly the on-line application form after payment Click on “SUBMIT” conclude the application process.

Applicants without an ATM Card should approach any bank for reloadable card and load the required amount to enable them pay on-line.

Federal University Otuoke Post UTME Screening Exercise 2013/2014

This is to inform the general public that the Post-UTME Screening Exercises of the Federal University, Otuoke, Bayelsa State, for the 2013/2014 session will take place as indicated below:

Examination/Screening Dates:

- 1st choice candidates: Saturday, July 20, 2013.

- 2nd choice candidates: Thursday, August 31, 2013.

Eligibility to Apply

Candidates who chose Federal University, Otuoke, as first and second choice ONLY. The Minimum UTME score for all candidates is 180.

1ST CHOICE CANDIDATES:

BEGIN ON WEDNESDAY, JULY 3, 2013.

END ON TUESDAY, JULY 16, 2013

2ND CHOICE CANDIDATES:

BEGIN ON, TUESDAY, AUGUST 20, 2013.

END ON TUESDAY, AUGUST 27, 2013

The Registration Process is as follows:

i. Go to any Diamond Bank, ECO Bank, UBA and Skye Bank branch in Yenagoa or any Diamond Bank and Skye Bank branch in Nigeria along with your UTME REGISTRATION SLIP.

ii. Purchase an e-pin slip for Post-UTME Screening Exercise for a fee of two thousand naira

(N2, 000.00) only. The University will not subsidize other charges such as Bank charges.

iii. Go online and log onto (www.fuotuoke.edu.ng) and link to the site (fuotuoke.ac) by clicking on Apply for Post-UTME Screening Exercise. Complete the online application for Post-UTME Screening.

iv. The Passport photograph: the passport photograph will be captured by using a web camera available on the computer you are using for the online registration. You do not need to scan and upload a photograph for the registration. Scanned passport photograph will render your application invalid. Candidates must ensure that the computer they are using has a web camera.

v. Print out the generated Acknowledgment Letter and your Application Form. Your seat number for the screening exercise will be indicated on the form. You must bring these (Acknowledgment Letter and Application Form) along to the screening test venue. Candidates are advised to make photocopies of the Acknowledgment Letter and Application Form.

Candidates without the Acknowledgment Letter and Application Form will not be allowed to take the Post-UTME Screening Test

vi. Notification for Post-UTME Exercise

You will receive a notification through SMS via telephone and by e-mail indicating whether or not you are qualified to do the screening test. The dates for your screening test will also be available on our website (www.fuotuoke.edu.ng). Also available on our website is a sketch of the screening test venue to enable candidates to identify their examination venues.

Candidate who chose fuotuoke as first and second chioce, and scored 180 and above in Jamb, should go to the following banks and purchase their e-pin at N 2000 only(diamond bank,eco bank,UBA and Sky bank).

Kindly note that...

i. Your application will not be successful if:

- your UTME score is less than 180.

- the appropriate e-pin was not used.

- if you enter a State of Origin different from the one indicated on your JAMB FORM.

Note also that the screening exercise starts from your buying of the e-pin.

ii. Always check your e-mail box for mails and the University website for up-dates.

Important Instructions

Punctuality to Screening

The screening process takes a lot of time because it involves biometric techniques such as finger printing. Late comers will not be screened.

Candidates who are resident outside Yenagoa are advised to come a day earlier. The University is not responsible for accommodating any candidate.

The Federal University Oye-Ekiti has scheduled its 2013/2014 Post-UTME Screening tests to hold from Wednesday 31st July to Friday, 9th August, 2013.

3.0 ELIGIBILITY

i) Candidates who obtained the UTME cut-off scores of 180 and above who chose Federal University Oye-Ekiti as their 1st or 2nd choice.

ii) Candidates who did not choose the Federal University Oye-Ekiti as their 1st or 2nd choice but obtained a minimumscore of 180 in UTME can also apply.

4.0 REGISTRATION FEE

Each candidates will be required to pay Two Thousand Naira (N2,000.00) only, excluding bank charges as Registration fee.

5.0 REGISTRATION PROCEDURE

i) Visit any bank and pay through e-Transact platform

ii) You will receive a print out with receipt number and confirmation order number.

iii) Visit Federal University Oye-Ekiti website www.fuoye.edu.ng and click on post-UTME linkfrom the homepage

iv) Go to payment portal section and insert the details received from the bank and this will enable you access to the Application Form.

Submit Application Online

Candidates are advised to download their POST-UTME Application Forms after submission and bring them to the venue of screening on the day of their examination.

i) Please use exactly the same names (and in the same

order) as used on your JAMB Registration Form;

ii) Submit a valid e-mail address and a valid personal cell phone number in your Registration Form. No two candidates are allowed to use same cell phone number.
